Preserve Harmony Decoction

Bohwa-tang

한글 보화탕  ·  Pinyin Bao He Tang

保和湯

Decocted with 3 slices of ginger to half, strained; a large spoonful of malt syrup is stirred in until melted, and taken after food, three times daily. The Sinseo appends a modification table: for exuberant blood add cattail pollen, madder, and lotus node; for exuberant phlegm add arisaema, pinellia, tangerine peel, bitter orange, and trichosanthes seed; for urgent panting add mulberry root bark, lepidium seed, and tangerine peel; for exuberant heat add gardenia, scutellaria lamina, and forsythia; for exuberant wind add saposhnikovia, schizonepeta, and inula; for exuberant cold add ginseng and cinnamon bark. Note on modern practice: aristolochia fruit (馬兜鈴) contains aristolochic acid and is prohibited in contemporary U.S. practice due to nephrotoxicity; modern prescribing omits or substitutes it — the formula stands as recorded.

Clinical commentary 臨床

Ge Kejiu's lung formula is an aviary of moisteners — nineteen light herbs, none over a don, finished with a spoon of syrup so the medicine coats on its way down. Built for the cough that has started bringing blood: everything soft, everything soothing, the era's palliative respiratory care. **馬兜鈴 is the fruit of 쥐방울덩굴, Aristolochia, and carries aristolochic acid — nephrotoxic, and a Group 1 carcinogen. 草部下 1.112 states the point that matters here: the acid is in the seed as well as the pericarp and the book's light dry-frying does not destroy it, so there is no preparation of this drug that makes it safe. It must be omitted.** This formula is the worst exposure pattern for it in the corpus — a decoction taken 日三次, three times a day, for 勞嗽, a cough that by definition does not resolve quickly. Aristolochic acid nephropathy is a cumulative injury, and a long course of small doses is the shape it takes. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
