Donguibogam 東醫寶鑑

消積塊專攻之劑. 大黃四兩(醋浸七日, 日曬夜露又七日), 穿山甲(土炒) 二兩, 木鱉子(去油)ㆍ香附子(便浸炒)ㆍ桃仁各一兩, 紅花二錢, 靑黛五分. 右爲末, 將大黃和好醋煮, 爲糊和丸菉豆大. 茅根葛根煎湯下五七十丸. 《回春》

Translation

A medicine that attacks accumulation lumps directly. Rhubarb 4 nyang, steeped in vinegar seven days, then sunned by day and dewed by night a further seven; pangolin scale (earth-fried) 2 nyang; momordica seed (oil removed), cyperus (child-urine steeped and fried) and peach kernel 1 nyang each; safflower 2 don; indigo 5 pun. Powder them, boil the rhubarb with good vinegar to make a paste, and form pills the size of a mung bean. Take fifty to seventy down with a decoction of imperata root and pueraria. — Manbyeong Hoechun 回春 (Donguibogam · Jeokchwi)

The chapter labels it as a medicine that attacks directly, which is its own warning — the section has just said at 1.7.4 that a lump should be brought down and then the patient greatly supplemented, and this formula does only the first half. Fourteen days of vinegar steeping and exposure for the rhubarb is unusually long and moderates its purgative violence while keeping the blood-breaking action. **Contains pangolin scale, a CITES Appendix I species prohibited in international trade; contemporary practice substitutes with medicinals such as 王不留行 or 皂角刺. Momordica seed is toxic.
