Large Gentian and Turtle Shell Powder

Jingyobyeolgap-san

한글 진교별갑산 · Pinyin Qin Jiao Bie Jia San

秦芃鼈甲散

The source glosses cheonseondeung in a side-note: 'the vine of green aucklandia' (靑木香藤也). Note on modern practice: Aristolochia vine (天仙藤) contains aristolochic acid and is prohibited in contemporary U.S. practice due to nephrotoxicity; the formula is shown as recorded, and modern prescribing omits or substitutes it.

Translation

Treats vacuity taxation with tidal heat, night sweats, and the various phlegm-cough patterns. Kudzu root 1.5 don; schizonepeta, fritillaria, peucedanum, aristolochia vine (the vine of green aucklandia), green tangerine peel, bupleurum, large gentian, turtle shell, and licorice, 0.75 don each; angelica dahurica, notopterygium, and cinnamon bark, 0.35 don each. Cut the above, make one packet, add 3 slices of ginger, decoct in water and take. — Deukhyo 得效 (Donguibogam · Heoro)

Clinical commentary 臨床

A consumption formula that fights outward — kudzu and schizonepeta at the gates while gentian and turtle shell dig the steaming heat out of the bone, on the Deukhyo's wager that taxation fever is a siege best broken from both sides. **天仙藤 is the stem of the same 쥐방울덩굴 whose fruit is 馬兜鈴, and carries the same aristolochic acid — nephrotoxic, Group 1 carcinogen. It must be omitted.** The leaf's own interlinear gloss names the plant for us: 天仙藤靑木香藤也, the 靑木香 vine. That is one of only three places in the corpus where this name is attached to the birthwort rather than to costus, and in all three the name is qualified — 土 for the root here and at 草部下 1.112.1, 藤 for the stem — while the bare 靑木香 is costus at 草部上 1.18. The formula is a decoction for 虛勞, taken over a long course, which is the exposure pattern aristolochic acid punishes. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
