破血散疼湯

Cut and divided into two packets; each dose is decocted in two cups of wine and one of water down to one cup, strained, and the powdered leech and musk stirred in, taken on an empty stomach. Two doses and it is cured. **Note on modern practice: leech (水蛭) is not used in contemporary practice; musk is a protected animal product and contemporary preparations substitute synthetic musk (muscone). Traumatic back injury with severe pain requires imaging to exclude fracture. Recorded as classical text.**

Donguibogam 東醫寶鑑

治墮落損傷, 跌其腰脊, 惡血留於脇下, 痛楚不能轉側. 水蛭 三錢(炒令烟盡, 另細硏), 連翹ㆍ當歸ㆍ柴胡 各二錢, 蘇木 一錢半, 羌活ㆍ防風ㆍ桂心 各一錢, 麝香 五分(另硏). 右剉, 分二貼, 每服, 酒二盞ㆍ水一盞, 煎至一盞去滓, 調水蛭ㆍ麝香末, 空心服, 兩服卽愈. 《東垣》

Translation

Treats injury from a fall, where the lumbus and spine have been struck, malign blood lodges beneath the flank, and the pain is such that one cannot turn to either side. Leech 3 don, dry-fried until the smoke ceases and separately ground fine; forsythia, Korean angelica, and bupleurum, 2 don each; sappan wood 1 don 5 fun; notopterygium, saposhnikovia, and cinnamon heart, 1 don each; musk 5 fun, separately ground. Cut the above and divide into two packets; for each dose decoct in two cups of wine and one of water down to one cup, strain, stir in the powdered leech and musk, and take on an empty stomach — two doses and it is cured. — Dongwon 東垣 (Donguibogam · Yo)

Clinical commentary 臨床

The most aggressive formula in the chapter, and reserved for the most specific situation: a fall that struck the lumbus, with blood now lodged under the flank and the patient unable to turn. Leech is the tradition's strongest blood-breaker and it leads here at three don. The construction is otherwise a wind-and-blood formula — bupleurum and sappan reach the flank, notopterygium and saposhnikovia the back — with musk added to drive the whole through. — Eui Young (Justin) Chung, L.Ac., Dipl.
