Bupleurum and Tangerine Peel Decoction

Sijin-tang

한글 시진탕  ·  Pinyin Chai Chen Tang

柴陳湯

The Bogam carries two different formulas under the name 柴陳湯: this one, from the Haehak chapter, for phlegm malaria, while the Hyung chapter's Immun formula of the same name treats phlegm heat with glomus fullness of the chest. The weighting is what separates them — here bupleurum and pinellia stand level at two don each, there bupleurum alone is raised and ginseng drops to seven fun — and this leaf writes 薑三棗二 without 入.

Clinical commentary 臨床

Sosiho-tang and Ijin-tang joined, the same join as the Chest chapter's formula of the name but re-weighted for the disease in hand: pinellia is lifted from a don to two so that it stands level with bupleurum, and the formula becomes as much a phlegm formula as a shaoyang one. 痰瘧 is the chapter's name for a malaria whose fit comes with vomiting of phlegm and a stifled chest rather than with plain shivering, and the chapter's view is that the phlegm is what keeps the fit returning. Hence the doubling, and hence red poria rather than white — the damp is to be moved downward and out. Of the chapter's many formulas this is the one with the shortest list and the clearest logic, which is also why it survived into later practice as the standard answer for a periodic fever with phlegm. **Contains pinellia at a doubled dose; it is toxic raw and must be properly processed before use.** — Eui Young (Justin) Chung, L.Ac., Dipl.
