Translation

Treats heat of every kind, and the heat of fright with agitation and restlessness. Ginseng, poria with wood, saposhnikovia, cinnabar and bupleurum, 2 don each; gold leaf thirty sheets. Powder the above and form with honey into pills the size of a paulownia seed; for each dose one pill, let down in bamboo sap. — Jikso 直小 (Donguibogam · Soa)

Clinical commentary 臨床

Two don of cinnabar in a batch of pills the size of a paulownia seed, with thirty sheets of gold leaf, 人參 and 茯神 to settle the spirit, and 柴胡 and 防風 for the heat. The gold is inert and does nothing; the cinnabar is the whole problem. 驚熱煩躁 — fever with fright and agitation — describes a child who is febrile and cannot be settled, and that is one of the presentations that should move a clinician fastest rather than toward a sedative. Irritability that does not settle, in a febrile child who cannot be consoled, is a sign of meningitis or encephalitis. The height of the temperature says little; how the child looks says most of it. See the chapter prose at 1.41 for the frame. **Contains cinnabar. The full statement on mercury in this chapter is written on `jinsa-go-soa`; read it there. In short, there is no safe dose at any age, the developing nervous system is the organ at risk, and the injury does not reverse. Here it is a fifth of the batch by weight, in a pill given to an infant, and the text gives no dose by weight anywhere. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
