Donguibogam 東醫寶鑑

治中脘有痰水, 心下汪洋嘈雜, 多唾吐淸水, 不欲食. 此胃虛冷也. 其脉沈弦細遲. 半夏ㆍ陳皮ㆍ旋覆花ㆍ人參ㆍ細辛ㆍ桂心ㆍ䓀莄ㆍ白芍藥ㆍ赤茯苓ㆍ甘草 各一錢. 右剉, 作一貼, 薑 五片, 煎服. 《東垣》

Translation

Treats phlegm-water in the middle duct, with a sense of open water below the heart and a gnawing discomfort, copious spitting and vomiting of clear fluid, and no desire for food. This is vacuity cold of the stomach. The pulse is sunken, wiry, fine and slow. Pinellia, tangerine peel, inula flower, ginseng, asarum, cinnamon heart, platycodon, white peony, red poria and licorice 1 don each. Cut up, make one packet, add 5 slices of ginger, decoct and take. — Dongwon 東垣 (Donguibogam · Damum)

Clinical commentary 臨床

汪洋 — a sense of open water below the heart — is the phrase worth carrying over intact. It describes the sloshing, unmoored feeling of fluid standing in the stomach, and paired with 嘈雜, the gnawing that is neither hunger nor pain, it gives a picture that maps closely onto gastric retention. All ten medicinals at exactly 1 don is unusual and marks this as a harmonizing formula rather than one with a single line of attack. Inula flower is the specific here: it downbears counterflow and dissolves phlegm-water, and it appears in the chapter's single-medicinal section as well. The section around it is a small differential worth reading. Copious spitting without cough in spleen vacuity that cannot restrain kidney water points to Palmi-won; after a serious illness it is a cold stomach and points to Ijung-tang; white froth means cold standing at the stomach mouth and alpinia is added.
