Clinical commentary 臨床

The leaf opens with a season and a population rather than with a patient: in the warm and hot months, when the people are ill with epidemic febrile disease, the heat should be cleared and the toxic qi resolved. Then eleven drugs, with notopterygium at two don at the head and gypsum, white peony and ginseng behind it at a don two fun five ri. The wine-processing is the signature. Scutellaria and anemarrhena are both wine-fried, coptis is wine-fried, raw rehmannia is wine-washed — four of the coldest drugs in the formula put through wine so that they rise and reach the surface instead of sinking straight to the bowel. That is Jeoljae's habit, and it is what lets a cooling formula still be given to someone with an exterior pattern, which is also why notopterygium leads it. Ginseng in the middle is the other deliberate choice: the heat is to be cleared, but not at the patient's expense. Of all the formulas in this chapter, this is the one whose reasoning is most visible in its processing instructions. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
