Donguibogam 東醫寶鑑

治傷寒病, 少陽轉屬陽明, 身熱ㆍ不惡寒ㆍ反惡熱ㆍ大便堅ㆍ小便赤ㆍ譫語ㆍ腹脹ㆍ潮熱. 柴胡 四錢, 黃芩ㆍ芍藥 各二錢半, 大黃 二錢, 枳實 一錢半, 半夏 一錢. 右剉, 作一貼, 入薑 三片, 棗 二枚, 水煎服. 《正傳》 ○小柴胡去人參ㆍ甘草, 加芍藥ㆍ大黃ㆍ枳實也. 以芍藥下安太陰, 使邪氣不納; 以大黃去地道不通; 以枳實去心下痞悶也. 《海藏》

Translation

Treats cold damage disease where the shaoyang turns over and belongs to the yangming, with generalized fever, no aversion to cold but rather aversion to heat, hard stool, dark urine, delirium, abdominal distension and tidal fever. Bupleurum, 4 don; scutellaria and peony, 2 don 5 fun each; rhubarb, 2 don; unripe bitter orange, 1 don 5 fun; pinellia, 1 don. Cut up, make one packet, add three slices of ginger and two jujubes and decoct in water. — Jeongjeon 正傳 ○ It is Minor Bupleurum with the ginseng and licorice removed and peony, rhubarb and bitter orange added. The peony settles the taiyin below so that the evil qi is not taken in; the rhubarb removes the blockage of the earthen path; the bitter orange removes the stuffiness and oppression below the heart. — Haejang 海藏 (Donguibogam · Han, upper)

Clinical commentary 臨床

Minor Bupleurum with the supporting drugs taken out and the purgatives put in, which is exactly what the pattern requires: the patient is no longer half-in and half-out but has gone inward, so ginseng and licorice — which hold a patient steady — would now be holding the evil in with them. Haejang's gloss on each substitution is worth keeping. Aversion to heat in place of aversion to cold is the single sign that marks the crossing over, and it is the one to check before choosing between this and the Minor. — Eui Young (Justin) Chung, L.Ac., Dipl.
