Angelica Pain-Draining Decoction

當歸拈痛湯

Cut and made into one packet; steep the medicines briefly in two cups of water, decoct down to one cup, and take one dose on an empty stomach and one at bedtime. The chapter records a case: an official campaigning in the south developed leg qi with the whole body slightly swollen and pain the hand could not approach, worst in the shin; three-edged needling of the swelling drew blood two cheok high, gradually thinning to a thread, black-purple in color, and shortly the swelling receded and the pain lessened; this formula was then given, he slept that night, and a second dose the next day cured it.

Donguibogam 東醫寶鑑

治濕熱脚氣腫痛. 羌活ㆍ茵陳(酒炒)ㆍ黃芩(酒炒)ㆍ甘草(灸) 各一錢, 知母ㆍ澤瀉ㆍ赤茯苓ㆍ猪苓ㆍ白朮ㆍ防己 各六分, 人參ㆍ苦參ㆍ升麻ㆍ乾葛ㆍ當歸ㆍ蒼朮 各四分. 右剉, 作一貼, 水二盞浸藥少時, 煎至一盞, 空心ㆍ臨臥各一服. 《寶鑑》

Translation

Treats damp heat leg qi with swelling and pain. Notopterygium, wine-fried virgate wormwood, wine-fried scutellaria, and roasted licorice, 1 don each; anemarrhena, alisma, red poria, polyporus, white atractylodes, and stephania, 6 fun each; ginseng, flavescent sophora, cimicifuga, kudzu root, Korean angelica, and atractylodes, 4 fun each. Cut the above, make one packet; steep briefly in two cups of water and decoct to one cup; take one dose on an empty stomach and one at bedtime. — Bogam 寶鑑 (Donguibogam · Jok)

Clinical commentary 臨床

One of the most enduring formulas in East Asian medicine, still in daily use for hot swollen joints of the lower limbs. Its design is unusual for a damp-heat prescription: notopterygium and artemisia lead, four draining herbs take the damp out through the urine, but ginseng, angelica, and atractylodes are in it as well, because the chapter's rule forbids purging a leg qi patient into vacuity.
