Donguibogam 東醫寶鑑

治痰氣鬱結, 窒碍於咽喉之間, 咯之不出, 嚥之不下, 謂之梅核氣者. 半夏ㆍ陳皮ㆍ赤茯苓 各一錢, 神麴(炒)ㆍ枳實ㆍ南星(炮) 各七分, 靑皮ㆍ厚朴ㆍ紫蘇葉ㆍ檳榔ㆍ縮砂 各五分, 白豆蔲ㆍ益智仁 各三分. 右剉, 作一貼, 入薑 五片, 水煎服. 《醫鑑》

Translation

Treats phlegm and qi depressed and bound, obstructing the space of the throat so that it will neither be coughed up nor swallowed down — what is called plum-pit qi. Pinellia, tangerine peel and red poria 1 don each; medicated leaven (stir-fried), bitter orange and arisaema (blast-fried) 7 pun each; green tangerine peel, magnolia bark, perilla leaf, areca and amomum 5 pun each; round cardamom and alpinia 3 pun each. Cut up, make one packet, add 5 slices of ginger, decoct in water and take. — Gogeum Uigam 醫鑑 (Donguibogam · Damum)

Clinical commentary 臨床

Sachil-tang — the Four Seven Decoction — is named for the seven affects it treats and the four medicinals it started from, and it is the classical prescription for plum-pit qi. What the augmentation adds is a whole digestive layer: medicated leaven, bitter orange, areca, amomum, round cardamom and alpinia. That is a formula for qi stuck in the middle burner grafted onto a formula for qi stuck in the throat, and it reflects the chapter's view that a lump felt at the throat usually has its root lower down.
