Translation

Cinnamon twig, cut up separately, and roasted licorice, 1 don 8 fun each; atractylodes, ginseng and dried ginger, 1 don 5 fun each. Cut up, make one packet; take two cups of water, decoct down to one cup, then put in the cinnamon twig, boil down to seven parts, strain and take warm, twice a day. — Junggyeong 仲景 (Donguibogam · Han, lower)

Clinical commentary 臨床

Regulate the Middle Decoction with cinnamon twig, and the instruction to cut the cinnamon separately and add it late is what makes it work. Cinnamon twig's surface-releasing action lies in its volatile oil, which a long decoction drives off; so the four interior drugs are decocted out fully and the cinnamon gets only the last few minutes. That single procedural choice lets one cup treat two levels at once. The pattern — diarrhoea from interior cold with a fever that will not clear — is common and usually mishandled in one direction or the other. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
