Donguibogam 東醫寶鑑

治濕痰. 半夏ㆍ南星各一斤, 爲末, 薑汁調作片, 置篩中, 以楮葉或艾葉覆之, 令發黃色, 曬乾收之, 須五六月間做麴. 每麴四兩, 入香附末二兩, 薑汁糊和丸梧子大, 薑湯下五十丸. 《入門》

Translation

Treats damp phlegm. Pinellia and arisaema 1 geun each, powdered and worked into cakes with ginger juice, set in a sieve and covered with paper mulberry or mugwort leaves until they turn yellow, then sun-dried and stored; the leaven must be made in the fifth or sixth month. To every 4 nyang of the leaven add 2 nyang of powdered cyperus, make pills with ginger juice paste the size of a firmiana seed, and take fifty down with ginger decoction. — Uihak Immun 入門 (Donguibogam · Damum)

Clinical commentary 臨床

A fermentation, not a mixture. Pinellia and arisaema are cakes covered with mulberry or mugwort leaves and left to go yellow, which is a controlled mould culture — the same principle as medicated leaven, applied to two toxic drying medicinals. The fifth or sixth month is specified because that is when the heat and humidity will actually raise the culture. What ferments is milder and more digestible than what went in. Cyperus is added afterward, at half the weight of the finished leaven, so the qi-coursing action sits on top of the transformed phlegm rather than being fermented along with it.
