Bupleurum and Cinnamon Twig Decoction

Sihogyeji-tang

한글 시호계지탕  ·  Pinyin Chai Hu Gui Zhi Tang

柴胡桂枝湯

The Bogam carries two different formulas under the name 柴胡桂枝湯: this one, from the Haehak chapter, for shaoyang malaria with cold and heat coming and going by turns, while the Cold Damage chapter's Junggyeong formula of the same name treats cold damage with stirred qi and a throbbing pain. The leaf writes 芍藥 without specifying red or white and it is registered here as white peony; it also writes 右剉一貼 without 作.

Clinical commentary 臨床

Sosiho-tang with cinnamon twig and peony added — which is to say the Sanghallon formula of the name, carried over into the malaria chapter at the chapter's own lighter weighting, bupleurum alone at two don and everything else at one. The chapter files it under 少陽瘧 beside Sihogagye-tang, and the reasoning is plain in the placement: the alternation of cold and heat is the shaoyang's own sign, so a malaria whose fit comes and goes by turns is treated where that sign belongs rather than by anything that attacks the fit itself. The chapter's rule above it is 瘧屬三陽宜汗宜吐 — malaria in the three yang answers to sweating and ejection — and this is the sweating half, the cinnamon twig opening outward while bupleurum turns the pivot. Read against modern medicine the periodic fever maps onto malaria readily enough, but nothing here is antimalarial; a genuine malaria needs an antimalarial drug and prompt diagnosis. **Contains pinellia, which is toxic raw and must be properly processed before use.** — Eui Young (Justin) Chung, L.Ac., Dipl.
