Translation

Treats cold damage with suspended rheum and hidden rheum and pain drawing below the ribs. Genkwa, lightly stir-fried, kansui and stir-fried euphorbia, in equal parts, powdered. Separately take ten large jujubes, boil in 1 cup of water down to half a cup, remove the jujubes, and stir the powder in — 1 don for the robust, half a don for the weak. When the stool loosens and the water comes down, restore him with gruel. Hagan says: the acridity of genkwa disperses the rheum and the bitterness of the other two drains the water; kansui goes straight to the place where the water qi is bound and is the sage medicine for draining water — but it is toxic and must not be used lightly. — Uihak Immun 入門 · Seonmyeongbang 宣明 (Donguibogam · Han)

Clinical commentary 臨床

**This must not be prepared.** Genkwa, kansui and euphorbia are all violently purgative and all three are toxic; Liu Wansu's own warning is quoted in the entry and should be read as final. What makes the formula worth recording is everything around the three drugs: the jujube decoction as the vehicle, chosen to protect a stomach about to be assaulted; the dose graded by whether the patient is robust or weak; and the instruction to feed him gruel as soon as the water comes down. Suspended rheum with pain below the ribs is a pleural effusion, and draining it was the one thing this pharmacy could sometimes do. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
