Donguibogam 東醫寶鑑

取膈上濕痰, 熱積, 上可取胸膈之痰, 下可利腸胃之痰. 惟胃虛少食者忌用. 又云, 治風痰ㆍ熱痰ㆍ濕痰ㆍ食積痰. 芫花(醋浸一宿炒黑)ㆍ甘遂(麪裹煨熟, 水浸半日曬乾)ㆍ大戟(長流水煮曬乾) 各五錢, 大黃(濕紙裹煨, 再用酒浸炒熟) 一兩, 黃柏(炒) 二兩. 右爲末, 粥丸麻子大, 每十丸, 臨臥, 津嚥下. 《丹心》 一方, 白朮膏和丸蘿葍子大, 臨臥, 津液嚥下二三十丸, 白湯送下.

Translation

Draws out damp phlegm and heat accumulation above the diaphragm; above it can take phlegm from the chest and diaphragm, below it can free phlegm from the intestines and stomach. Only those with a vacuous stomach and poor appetite must avoid it. It is also said to treat wind phlegm, heat phlegm, damp phlegm and food accumulation phlegm. Genkwa (steeped overnight in vinegar and stir-fried black), kansui (wrapped in dough and roasted through, then steeped in water half a day and sun-dried) and euphorbia (boiled in running water and sun-dried) 5 don each; rhubarb (wrapped in damp paper and roasted, then wine-steeped and fried through) 1 nyang; phellodendron (stir-fried) 2 nyang. Powder them, make congee pills the size of a hemp seed, and take ten at bedtime, swallowed with saliva. One version binds them with atractylodes paste into pills the size of a radish seed, twenty or thirty swallowed with saliva at bedtime and sent down with plain hot water. — Dansim 丹心 (Donguibogam · Damum)

Clinical commentary 臨床

The three drastic purgatives of the Ten Jujube Decoction — genkwa, kansui and euphorbia — with rhubarb and phellodendron added, and every one of the three processed at length before use. Vinegar for the genkwa, a dough jacket for the kansui, running water for the euphorbia: three different methods because each carries its toxicity differently. The contraindication is stated plainly and is the important line: only those with a vacuous stomach and poor appetite must avoid it. The dose of ten hemp-seed pills, swallowed with saliva rather than washed down, is deliberately small. **This formula contains three toxic drastic purgatives. It is not suitable for self-administration, is contraindicated in pregnancy, and all three chief medicinals are classically incompatible with licorice.** Augmented Minor Stomach Elixir at 6.13.1 is built on this base. — Eui Young (Justin) Chung, L.Ac., Dipl.
