Donguibogam 東醫寶鑑

治太陽經中寒濕, 肩背痛, 不可回顧. 又云, 肩背痛, 乃風熱乘肺, 肺氣鬱甚也. 黃芪ㆍ升麻ㆍ柴胡 各一錢, 防風ㆍ羌活ㆍ陳皮ㆍ人參ㆍ甘草 各五分, 靑皮 三分, 白豆蔲ㆍ黃柏 各二分. 右剉, 煎服. 《東垣》

Translation

Treats cold and damp striking the greater yang channel, with pain of the shoulder and back and inability to turn and look behind. It is also said: shoulder and back pain is wind-heat riding the Lung, with Lung Qi severely constrained. Astragalus, cimicifuga and bupleurum 1 don each; siler, notopterygium, aged tangerine peel, ginseng and licorice 0.5 don each; green tangerine peel 0.3 don; round cardamom and phellodendron 0.2 don each. Clinical commentary 臨床

Li Dongyuan's signature construction: astragalus with cimicifuga and bupleurum to raise clear yang, which is why the formula treats a back that hurts without treating the back directly. The text gives two readings of the same complaint in succession — cold-damp in the greater yang, and wind-heat constraining the Lung — and the formula answers both, with the wind medicines addressing the first and the Qi-raising core the second. The tiny quantities of green tangerine peel, cardamom and phellodendron are corrective rather than therapeutic: they keep the raising action from becoming a dry ascent.
