Donguibogam 東醫寶鑑

治脾積. 黃連 八錢, 厚朴 四錢, 吳茱萸 三錢, 黃芩 二錢, 縮砂 一錢半, 白茯苓ㆍ人參ㆍ澤瀉 各一錢, 茵蔯ㆍ乾薑 各一錢半, 川烏ㆍ川椒 各五分, 桂皮ㆍ巴豆霜 各四分, 白朮 二分. 右爲末, 蜜丸梧子大. 甘草湯下, 服如上法. 《正傳》

Translation

Treats spleen accumulation. Coptis 8 don; magnolia bark 4 don; evodia 3 don; scutellaria 2 don; amomum 1.5 don; white poria, ginseng and alisma 1 don each; artemisia and dry ginger 1.5 don each; aconite and zanthoxylum 5 pun each; cinnamon bark and croton frost 4 pun each; white atractylodes 2 pun. Powder them, make honey pills the size of a firmiana seed, and take down with a decoction of licorice, dosing as above. — Uihak Jeongjeon 正傳 (Donguibogam · Jeokchwi)

Clinical commentary 臨床

Artemisia and alisma are the spleen-specific additions and they answer the indication directly: the pattern description at 1.2 names jaundice among the signs of spleen accumulation, and artemisia is the classical jaundice medicinal. White atractylodes at 2 pun is almost token — striking in a spleen formula — because the mass is being attacked and the spleen supported only enough to bear it. The phrase 飮食不爲肌膚, food and drink not becoming flesh, is the one worth carrying: eating without gaining, which the Phlegm-Rheum chapter also uses as a marker at 6.7. **Contains croton frost and aconite, both toxic.
