Donguibogam 東醫寶鑑

治心積. 黃連一兩半, 厚朴ㆍ人參各五錢, 黃芩ㆍ桂枝ㆍ茯神ㆍ丹參各一錢, 乾薑ㆍ菖蒲ㆍ巴豆霜ㆍ川烏各五分, 紅豆蔲二分. 右爲末, 蜜丸梧子大. 黃連湯下, 服如上法. 《正傳》

Translation

Treats heart accumulation. Coptis 1.5 nyang; magnolia bark and ginseng 5 don each; scutellaria, cinnamon twig, root poria and salvia 1 don each; dry ginger, acorus, croton frost and aconite 5 pun each; galangal fruit 2 pun. Powder them, make honey pills the size of a firmiana seed, and take down with a decoction of coptis, dosing as above. — Uihak Jeongjeon 正傳 (Donguibogam · Jeokchwi)

Clinical commentary 臨床

Coptis at a nyang and a half, three times the liver formula's dose, which is the matrix at 1.8.1 applied: the higher the accumulation, the hotter it is read. Root poria, salvia and acorus are the heart-specific additions, and all three quiet the spirit rather than break the mass — the formula treats where the accumulation sits as much as what it is. Taking it down with a decoction of coptis rather than warm water reinforces the same direction. Note that 1.3 records a second and quite different 伏梁, wrapped in pus and blood outside the intestines, which the Neijing says must not be treated — pressing it firmly kills. The two share a name and are not the same disease. **Contains croton frost and aconite, both toxic.
