Translation

After sweating or purging in cold damage, there is depletion vexation and sleeplessness; in severe cases the patient necessarily tosses and turns over and over with a gnawing distress in the heart. This governs it. — Junggyeong 仲景 ○ Where pressing below the heart finds it soft, it is depletion vexation. Gardenia, seven; fermented soybean, half a hop. Cut up; take two cups of water and first decoct the gardenia down to one cup, add the soybean, decoct again down to seven parts, strain off the dregs and take warm. If vomiting is obtained, stop; if there is no vomiting, take again. — Junggyeong 仲景 ○ If the chest is full and the qi short, add licorice; this is called Chiji-gamcho-tang 梔豉甘草湯. ○ If the chest is full and there is retching, add fresh ginger; this is called Chiji-saenggang-tang 梔豉生薑湯. — Immun 入門 ○ Ononong 懊憹 is the heart depressed and not at ease. The pattern may arise from purging in error: the upright qi is emptied within, the yang evil falls inward and binds in that space; in severe cases it becomes a chest bind. — Hoechun 回春 (Donguibogam · Han, upper)

Clinical commentary 臨床

Two ingredients and one of the most precise differential tests in the canon: press below the heart, and if it is soft this is depletion vexation and belongs here, while if it resists it is a chest bind and belongs to the Chest-Draining formulas. Hoechun's note explains the relation — the two are the same process at different depths, and purging in error is what turns one into the other. The gardenia is decocted first and alone, then the soybean goes in for a shorter time; both are light, venting drugs and a long boil would flatten them. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
