Donguibogam 東醫寶鑑

治胸中, 胃脘至咽門, 窄狹如線疼痛, 及手足俱有核如胡桃者. 䓀莄ㆍ便香附子ㆍ白殭蚕(炒) 各一錢, 陳皮ㆍ片芩ㆍ枳殼各七分, 前胡ㆍ半夏ㆍ枳實ㆍ羌活ㆍ荊芥ㆍ檳榔ㆍ射干ㆍ威靈仙各五分, 木香ㆍ甘草各三分. 右剉, 作一貼, 薑三片, 水煎服. 《醫鑑》

Translation

Treats narrowing from the chest and stomach duct up to the gate of the throat, as though drawn to a thread, with pain; and nodules like walnuts in both the hands and the feet. Platycodon, child-urine-processed cyperus and silkworm (stir-fried) 1 don each; tangerine peel, scutellaria in broad lamina and bitter orange 7 pun each; peucedanum, pinellia, bitter orange fruit, notopterygium, schizonepeta, areca, belamcanda and clematis 5 pun each; costus and licorice 3 pun each. Cut up, make one packet, add 3 slices of ginger, decoct in water and take. — Gogeum Uigam 醫鑑 (Donguibogam · Damum)

Clinical commentary 臨床

The indication is unusually vivid — a narrowing running from the chest up through the stomach duct to the throat, as though the passage had been drawn down to a thread. Silkworm, belamcanda and clematis are the three medicinals that mark this out from the other lump formulas: silkworm dispels wind phlegm and softens nodules, belamcanda is the classical throat medicinal for phlegm heat, and clematis frees the channels and is specific for what lodges along them. Notopterygium and schizonepeta open outward, which is the wind-phlegm reading of a nodule that came to rest where it did rather than growing there. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
