定喘湯

A different formula from the 定喘湯 of the Wheezing section, the Seopsaeng Jungmyobang's ginkgo-and-ephedra formula, which this chapter also carries. The weights here are given inside a mnemonic verse and cannot be assigned to the drugs with certainty; the figures are recorded in the preparation notes as the verse gives them, and the Bogam's own closing remark says the nyang and don of this version disagree with those of 人參定喘湯 and should be checked. The transcription reads 用生薑 where the first edition reads 角生薑, and 鸎粟 for 罌粟.

Donguibogam 東醫寶鑑

治肺虛久喘. 歌曰, 和劑須投定喘湯, 阿膠半夏及麻黃, 人參四兩同甘草, 四兩桑皮五味強, 鸎粟二錢須蜜灸, 三錢煎服用生薑, 多年氣喘從今愈, 始信良醫有妙方. 《醫鑑》 ○此方與上人參定喘湯同, 但兩錢之數異, 宜考.

Translation

Treats long-standing panting from lung vacuity. The verse runs: for a harmonizing prescription you must give Arrest Wheezing Decoction — donkey-hide gelatin, pinellia and ephedra; ginseng at four nyang together with licorice; four nyang of mulberry root bark and schisandra for strength; poppy husk, 2 don, must be honey-roasted; take 3 don decocted and use fresh ginger; the panting of many years is cured from today, and one comes to believe that a good physician has a wonderful prescription. ○ This formula is the same as Ginseng Arrest Wheezing Decoction above, except that the figures in nyang and don differ, and the matter should be looked into. — Gogeum Uigam 醫鑑 (Donguibogam · Haesu)

Clinical commentary 臨床

A verse formula, learnt by heart rather than read off a page, and the Bogam prints it with the warning that its figures do not add up. Four nyang of ginseng in a single prescription is an enormous quantity, and the parallel formula it points to has the same drugs in don. What the drugs say is clear enough even when the weights do not: gelatin, schisandra and poppy husk to hold, ephedra and pinellia to open and dry, ginseng and licorice beneath them. It is a formula for a wheeze of long standing in a patient too depleted for dispersing alone. **Contains poppy husk, an opium-containing material and a controlled substance, and ephedra, a sympathomimetic.
