Translation

Treats the grasping-at-emptiness pattern. This arises because liver heat rides the lung and the original qi is vacuous and weak and cannot hold, so that there is delirious speech, a clouded spirit, the hands crossed over the heart, or grasping at emptiness and fumbling at the bed. Ginseng, tangkuei, peony, bupleurum, scutellaria, atractylodes, ophiopogon, tangerine peel, poria spirit and licorice, 1 don each. Cut up, make one packet, add three slices of ginger and two jujubes, add refined gold and decoct together. — Uihak Immun 入門 (Donguibogam · Han)

Clinical commentary 臨床

The sign the formula is named for is one of medicine's oldest and most reliable: 撮空摸床, grasping at the air and picking at the bedclothes. Carphologia in a delirious patient is a grave sign and the tradition read it correctly as the original qi failing rather than as heat alone — hence a formula of ten supporting drugs with only bupleurum and scutellaria for the heat. Gold decocted with the drugs is a weighting agent in the elixir tradition and contributes nothing; the rest is a mild and sensible prescription for a patient whom nothing was going to save. — Eui Young (Justin) Chung, L.Ac., Dipl.
