滾痰丸

Donguibogam 東醫寶鑑

治濕熱痰積, 變生百病. 大黃(酒蒸)ㆍ黃芩(去朽) 各八兩, 靑礞石 一兩(同焰硝一兩入罐內盖定, 鹽泥固濟曬乾, 火煆紅, 候冷取出, 以礞石如金色爲度), 沈香 五錢. 右爲末, 滴水和丸梧子大, 茶淸, 溫水任下四五十丸. 服藥必須臨睡, 送下至咽, 卽便仰臥, 令藥在咽膈之間徐徐而下. 漸逐惡物入腹入腸, 方能見效.

Translation

Treats damp-heat phlegm accumulation transforming into a hundred diseases. Rhubarb (wine-steamed) and scutellaria (decayed parts removed) 8 nyang each; chlorite schist 1 nyang, sealed in a crock with 1 nyang of niter under a salt-clay lute, sun-dried, calcined until red and left to cool — correct when the schist has taken a gold colour; aloeswood 5 don. Powder them, make pills with dripped water the size of a firmiana seed, and take forty or fifty down with clear tea or warm water. The medicine must be taken at bedtime: send it down as far as the throat and lie back at once, so that it passes slowly through the throat and diaphragm, gradually driving the foul matter into the belly and bowels. The formula that the whole chapter is arranged around. Wang Eun-gun's phlegm treatise at 6.3 argues that the hundred strange diseases all issue from phlegm — the chapter calls this an insight no earlier writer had reached — and he composed this single prescription to treat the lot of them. The administration instruction is unusual and specific: take it at bedtime, get it as far as the throat, then lie flat at once so it descends slowly through the throat and diaphragm rather than dropping straight to the stomach. The formula is meant to work on the way down, sweeping the passage, which is what 滾 — to roll — refers to. **Contains a strong purgative at 8 nyang together with a calcined mineral. Not suitable for self-administration and contraindicated in pregnancy.** The chapter's own dosing table runs from thirty to a hundred pills depending on presentation, which is a very wide range and a further reason this belongs in trained hands. — Eui Young (Justin) Chung, L.Ac., Dipl.
